Tongkat Ali and the Liver: What NIDDK LiverTox Reports
LiverTox, the NIH liver injury database, gives tongkat ali a likelihood score of D, a possible rare cause of liver injury. It notes one case in a bodybuilder where undisclosed steroid use is an alternative explanation, and a 2021 EFSA opinion that safety has not been established. SodaPeak lists tongkat ali second, with no amount.

What does LiverTox say about tongkat ali?
That it is a possible rare cause of liver injury, with a likelihood score of D.
Tongkat ali is Eurycoma longifolia, also called Long Jack, a Southeast Asian tree. LiverTox says it is sold in the U.S. as an over-the-counter supplement promoted for testosterone and athletic performance (NIDDK LiverTox, Tongkat Ali). This site repeats that as a description of how it is sold, not as a claim.
| Point | Note |
|---|---|
| Liver injury score | D: possible rare cause |
| Case report | One bodybuilder; undisclosed steroid use is an alternative explanation |
| Traditional dose | 100 to 400 mg a day |
| Online products | Often 1,000 to 1,600 mg |
| EFSA, 2021 | Safety not established under any condition of use |

What does a D score mean?
That a link to liver injury is possible but not well established. It is a caution, not a diagnosis of risk for any one person.
The same page says human studies show mild rises in total testosterone over weeks with no change in free testosterone, not linked to better athletic performance. This site makes no claim from that, and it does not say SodaPeak does the same.

What does the SodaPeak label say?
Tongkat ali powder is the second name, with a ceiling of 600 mg per serving and no stated amount.
The 600 mg figure is arithmetic from the blend rule (21 CFR 101.36 (eCFR)), not a dose. The tongkat ali page covers the plant, and the side effects page covers the rest.
